Is Big Brother watching the right KPIs?

The DVSA has a few problems at the moment and its answer to problems is generally more legislation or, in their terms, guidance.
The rate of operators’ licences being revoked, curtailed, suspended etc. only seems limited by its human resources. Now it is keen on using computers to determine the ability of operators to run safely.
The KPI for transport must be the number of deaths and serious injuries on our roads on a daily and annual basis.
Operators may be falling foul of the myriad regulations related to road transport, but what is their safety record?
How many operations have been closed down with massive consequences to all concerned when their accident record was exemplary?
Maybe we need to ask?
